The monitoring and response plans on site are not approved by the EHD. The monitoring and <br />response plans must be current and approved by the EHD. Complete and submit a copy of the <br />monitoring and response plans for approval by the EHD within 30 days of receiving this report. An <br />approved copy will be returned for your records. A current copy was reviewed and approved. A copy <br />was received for records. <br />305. The Bravo box float and chain for the diesel pipe in the dispenser 5/6 failed to stop the flow of <br />product at the dispenser when tested. All monitoring equipment shall be maintained to activate an <br />audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. Correct <br />immediately by having a properly licensed, trained, and certified contractor replace the failed component <br />with a functional component (LG 113-x listed, if applicable) and obtain a permit within one business day <br />from the EHD. If the failed component can't be replaced immediately, there is a possibility that the <br />diesel UST system may be red tagged to prevent fuel inputs. <br />305. The Bravo box float and chain for the diesel pipe in dispenser 1/2 failed to stop the flow of product <br />at the dispenser when tested. All monitoring equipment shall be maintained to activate an audible and <br />visual alarm or stop the flow of product at the dispenser when it detects a leak. The service technician <br />adjusted the chain and verified functionality. <br />310. Liquid was observed in the Diesel and 91 STP sumps. If water could enter into the secondary <br />containment by precipitation or infiltration, it must be removed and disposed of properly. Immediately <br />remove this liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, <br />and manage it accordingly. Ensure that the all sump is maintained free of liquid. Technician removed <br />the water and put it in the facility's hazardous waste drum. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />Inspector Received By: Title: + r <br />t -v^ .
