The 91 float and the diesel float in UDC 3/4 and 1/2 failed to stop the flow of product at the dispenser <br />when tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or <br />stop the flow of product at the dispenser when it detects a leak. Correct immediately by having a <br />properly licensed, trained, and certified contractor replace the failed component with a functional <br />component (LG 113-x listed, if applicable) and obtain a permit within one business day from the EHD. If <br />the failed component can't be replaced immediately, there is a possibility that the 91 and diesel UST <br />system may be red tagged to prevent fuel inputs. Note: Diesel float in UDC was adjusted during MC to <br />function properly and the 91 float was replaced and tested. Permit must be obtained from this office <br />within 1 business day or a double permit fee will apply. <br />18. The 87 spill container failed when tested. All spill containers shall have a minimum capacity of five <br />gallons and be capable of containing a spill or overfill until it is detected or cleaned up. Immediately <br />discontinue deposition of petroleum into this tank until the component is repaired or replaced by a <br />properly licensed, trained, and certified contractor under permit and inspection of the EHD. If the spill <br />container can't be replaced immediately, there is a possibility that the 87 UST system may be red tagged <br />to prevent fuel inputs. Product level= W991 <br />21. Liquid was observed in the 87, and diesel fill sumps. If water could enter into the secondary <br />containment by precipitation or infiltration, it must be removed and disposed of properly. Immediately <br />remove this liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, <br />and manage it accordingly. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4.
