UST facility and tank forms on file with the EHD are not current. Any change of information on these <br />forms must be submitted to the EHD within 30 days of the changes. Immediately complete and submit a <br />copy of the facility and tank forms to the EHD. Complete a separate tank form for each tank. <br />6. Approved copies of the monitoring and response plans were not found on site. Copies of these plans <br />shall be retained on site at all times. Immediately locate and retain a copy on site. <br />7. Facility failed to record alarm on 4/20/10 for L4 (dsl turbine sump). Facility must keep record of all <br />alarm that occurred for 3 years and also note how the alarm was corrected. Immediately locate the <br />alarm log for 4/20/10 and submit to this office by 8/13/10. <br />21. Liquid was observed in the 87, 91, and diesel turbine sumps. If water could enter into the <br />secondary containment by precipitation or infiltration, it must be removed and disposed of properly. <br />Immediately remove this liquid, make a hazardous waste determination per Title 22 hazardous waste <br />regulations, and manage it accordingly. Ensure that the87, 91, and diesel sumps are maintained free of <br />liquid. <br />Notes <br />A copy of the return to compliance certification must be submit to this office with the requested <br />documents by 8/13/10. <br />Facility's financial responsibility is due by 7/20/10 <br />Provided free CUPA class information. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($115). <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Received By: Title: <br />EHD 23-02-003 Rev 06/01/10 CONTINUATION FORM <br />
