Each item is discussed below: Item#103:The site has more than one address associated with it.The address listed for the site's financial responsibility has been updated in CERS to represent the address your agency has on file for the site. Item#105: The fill tube shut-off valve has been identified as the primary overfill in the UST monitoring plan updated in CERS. Item#201: Sunwest has indicated that the 2016 monitoring system certification was performed on 9-1-2016 not 9-2-2016 as the copy onsite shows. Sunwest also has indicated that on 9-1-2016,the Sunwest technician entered the wrong date on the monitoring system certification.The corrected report is enclosed.Page 2 of the designated operator inspection reports also demonstrates compliance with monthly inspections of the unburied piping Item#204: This is a suction system and has no leak detector.The updated monitoring plan in CERS has been revised to remove electronic line leak detector performs 3 GPH leak test. Item #209: According to Sunwest, the monitoring system certification was performed on 9-1-2016, not 9-2-2016. The corrected report is attached and the monthly inspection reports reflect the 9-1-2016 date.
