Financial responsibility documents have not been submitted to the EHD. Current financial responsibility documents are required to be submitted annually. Annual monitoring system certification, leak detector testing, and spill container testing was performed on 9-11-12 and a test report was submitted to the EHD on 10-24-12, more than 30 days after the test. A copy of the test report must be submitted within 30 days of the tests. Ensure that a copies of test reports are submitted to the EHD within 30 days of the tests. 113. A new designated operator Reuben Becerra was hired on 9-11-12 and notification was not provided to the EHD within 30 days of the change. Any changes shall be reported to the EHD within 30 days. Immediately, submit notification to the EHD identifying all the designated operators for this facility. Alternatively, log into CERS, upload the required information, and submit for review by the EHD. During the inspection, the designated operator Spencer Kissick provided a current list of the facility designated operators including Ruben Becerra. Ensure that the EHD is notified within 30 days of any new facility designated operators. 502. A log of monthly checks of the unburied single walled piping was not found on site for the months of September 2012 through December 2012. For an emergency generator tank system, the unburied piping is exempt from regulation if visual inspections are made each time the system is operated, but no less than monthly, and a log is maintained. Immediately begin inspecting the system each time the system is operated, but no less than monthly, and maintain a log of the inspections, or submit a permit application to upgrade the UST system to meet Title 23 UST regulations. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
