PDF <br />,JR , <br />DOCYN (4 MB) <br />Hi Aris, <br />I am sending this e-mail to follow up on the 9/11/2012 inspection of MCI 13850 Devries <br />Road, Lodi, CA. Attached documentation includes a signed return to compliance <br />certification, hazardous waste manifest, bill of lading, owner statement of DO and the <br />associated inspection report. The following are compliance statements: <br />Item #305 - The petroleum-based material and debris has been cleaned up by contractor <br />Clean Harbors and the attached hazardous waste manifest is documentation of proper <br />disposal of the waste material from the clean up. <br />Item #804 and 809 - The used lamps were picked up as Universal Waste by Clean Harbors. <br />Also, local personnel were trained regarding the proper storage of used lamps and the <br />storage time limit for Universal Waste. <br />Item #113 - The owner statement of DO is part of the attachments. <br />Please let me know if you need any additional information or documentation.
