A new designated operator performed the February 16, 2012, designated operator monthly <br />inspection and completed the designated operator monthly inspection report. Notification was not <br />provided to the EHD within 30 days of the change. The EHD received a notification for the new <br />designated operator on May 14, 2012. Any changes shall be reported to the EHD with 30 days.
