Main Street <br />Stockton, CA 95202-3029 <br />Re: Underground Storage Tank Inspection at 13850 N. DeVries Road, Lodi, CA <br />Dear Mr. Cacapit: <br />On September 14, 2011 you completed an Underground Storage Tank and Hazardous <br />Materials Inspection at the MCI, doing business as, Verizon Business facility located at 13850 <br />N. DeVries Road in Lodi, CA. As part of the inspection, you listed the following items that <br />required immediate attention. Below is my response for each item: <br />Item #204: Designated Monthly Inspection Dates for October 2010: The designated operator <br />failed to check and note the correct dates for the monitoring system certification testing, <br />secondary containment testing and spill containment testing on the October 13, 2010 <br />designated operator monthly inspection report. I have enclosed a copy of the corrected October <br />2010 designated operator monthly inspection report. <br />Item #205: Designated Monthly Inspection: The UST designated monthly inspection for <br />September 2010 was not completed by Gary Duvall. Gary was the MCI, doing business as, <br />Verizon Business designated operator for this location. Due to a confidential internal company <br />human resources issue, Gary was placed on temporary leave in mid-August 2010. Since this <br />was a confidential internal human resources issue, Gary was only allowed to speak with his <br />manager and human resources. Following an internal company investigation, Gary was <br />terminated from MCI, doing business as, Verizon Business on September 3, 2010. With that <br />being the case, Gary was not able to complete his September 2010 inspection. EPIC <br />Compliance was retained to perform the monthly DO inspections and initiated this work in <br />October 2010. <br />
