De Vries Rd Lodi, CA 95240 <br />Program: UST <br />SUMMARY OF VIOLATIONS <br />CLASS I CLASS 11, or MINOR -Notice to Com <br />On site to conduct routine inspection and witness the annual monitoring system certification testing. <br />9. A small amount, about half a cup of red dye diesel was at the bottom of the piping/transition sump <br />next to the generator building. The underground storage tank system shall be operated to prevent <br />unauthorized releases. Immediately have a properly licensed, trained, and certified contractor trouble <br />shoot the leak and if needed, replace or repair the failed piping under permit and inspection of the EHD. <br />If the failed component can't be replaced immediately, there is a possibility that the UST system may be <br />red tagged to prevent fuel inputs. Inform this office of what will be done to address this issue. <br />14. A small amount, about half a cup of red dye diesel was at the bottom of the piping/transition sump <br />next to the generator building. All primary containment must be product tight. Immediately have a <br />properly licensed, trained, and certified contractor trouble shoot the leak and if needed, replace or repair <br />the failed piping under permit and inspection of the EHD. If the failed component can't be replaced <br />immediately, there is a possibility that the UST system may be red tagged to prevent fuel inputs. Inform <br />this office of what will be done to address this issue. Repeat Violation. <br />22. A small amount, about half a cup of red dye diesel was at the bottom of the piping/transition was not <br />recorded. Any release from the primary containment that does not escape the secondary containment <br />and was cleaned up within eight hours of being detected shall be recorded with the following information: <br />1. Operator's name and phone number <br />2. List of types, quantities, and concentrations of hazardous substances released <br />3. Description of actions taken to control and clean the release <br />4. Method and location of disposal of the released substances <br />5. Description of actions taken to repair and prevent future releases <br />6. Description of method used to reactivate interstitial monitoring after repair or replacement of <br />primary containment <br />Immediately record this release with the information required, and maintain it on site with the facility's <br />monitoring records. Submit a copy of the recordable release statement to the EHD by 10-13-10. Repeat <br />Violation. Blank copy of the Recording Requirements for Unauthorized Releases form was provided. <br />#23c The designated operator monthly inspection report for August 2010 was not found on site. <br />Designated operator monthly inspection reports for the previous twelve months shall be retained on site. <br />Locate and ensure that copies of the previous twelve months of designated operator monthly inspection <br />reports are maintained on site. Submit a copy of the August 2010 designated operator monthly report to <br />EHD by 10-23-10.
