Devries Road in Lodi, <br />CA. Paperwork issues and sensor work was completed on site during your inspection. <br />There was also an issue with the third party Designated Tank Operator (DTO), Afforda <br />Test, not properly completing the monthly DTO inspection form. Specifically, Afforda <br />Test was not listing the last successful SB989 test date. Company representatives have <br />spoken with Afforda Test and informed them they must be more diligent in completing <br />the monthly DTO inspection form.
