Fremont, Stockton CA 95215 (SR0058758) <br /> The San Joaquin County Environmental Health Department(EHD)has completed the <br /> review of the application for permit to replace the defective leak detector of the diesel <br /> grade underground storage tank(UST)with a Vaporless 99 LD-2000 mechanical line <br /> leak detector. The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution <br /> Control District for any applicable permits and requirements. <br /> 2. Contact Jeffrey Wong at(209)468-0335 or Kim Blackwell at(209)468-3427 <br /> at least 48 business-hours in advance to arrange an inspection to verify that <br /> the replaced line leak detector will detect a leak of 3 gallons per hour at 10 <br /> PSI. An inspection request in writing is also required(Fax: 209-468-3433). <br /> 3. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all <br /> of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by <br /> the Contractors State License Board(may be issued to the installation <br /> company), and <br /> b. A valid, current California UST Service Technician certificate issued by <br /> the International Code Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s)of the <br /> component(s)being tested, repaired, or serviced, and <br /> d. Each person installing or testing any component must hold a certificate of <br /> training from the manufacturer of that component. <br /> 4. Permit expires 180 days from date of approval. Note: If an extension to the <br /> expiration date is required, a written request must be submitted to this office prior <br /> to the expiration date. <br /> 5. All components must be certified by an independent testing organization and <br /> labeled as required by the organization. (UL, ULC,ANSI,API,ASTM, etc. See <br /> Page 1 of 2 <br />
