Contact the fire department for their permit requirements. <br /> 2. Samples must be taken and analyzed as stated on pages 4 and 5 of the permit application. <br /> 3. EHD must witness the soil sampling. Contact EHD to arrange this inspection appointment. <br /> 4. All removed piping must be decontaminated prior to removal. <br /> 5. See permit application for any additional changes. <br /> Conditions for sump,piping, and under-dispenser containment install: <br /> 1. Contact the local fire department and San Joaquin Valley Air Pollution Control District for <br /> applicable permits. <br /> 2. At least one person who holds the ICC UST Installer/Retrofitter Certification must be onsite when <br /> UST components are being installed. Additionally, the individual must have certification from the <br /> manufacturer of the equipment. <br /> 3. A third party certified tank and line integrity test must be performed once concrete is poured. Results <br /> must be received by EHD within 7 days of completing the job. <br /> 4. All piping and connections (including penetrations) shall bear a UL label. <br /> 5. All piping must slope to a containment sump with an audible and visual alarm. <br /> 6. All equipment in contact with backfill must have corrosion protection. <br /> 7. Contact EHD for the following inspections prior to backfill: <br /> a. 24 hour or equivalent lake test on the under-dispenser containment and tank sumps <br /> b. Primary piping inspection <br /> c. Secondary piping inspection <br /> d. Slope test of piping <br /> 8. Any changes in information or scope of work must be submitted to EHD prior to commencing work. <br /> 9.
