Sutter Tracy <br /> Community Hospital did contact the Board of Equalization and we were told that our BoE <br /> number had been closed out in 1990. <br /> In an unrelated conversation Sutter Tracy Community Hospital had with the Department of Toxic <br /> Substance Control(DTSC),the topic of Board of Equalization number came up.As we spoke <br /> with the DTSC representative,we were told that Sutter Tracy Community Hospital did not need a <br /> BoE number because our hospital generates less than five tons of hazardous waste a year.As <br /> such,we aboandoned our efforts to seek this information as it appeared, at that time,that there <br /> was no need for it. <br /> Sutter Health is currently amids a corporate name change and Sutter Tracy Community Hospital <br /> is waiting for the process to conclude so that we may upload this new information onto the CERS <br /> website.We were planning on excluding the BoE information within the UST Facility Operating <br /> Permit Application form. <br /> Unbeknownst to us, Sutter Tracy Community Hospital was unaware that a separate and distinct <br /> BoE is required for petroleum type applications.Regulatory agencies,often time,request similar/ <br /> identical type information on their forms(i.e. SIC Code,FEIN, etc.)which we submit as being <br /> accurate information. <br /> In light of this new information provided by San Joaquin County EHD,Sutter Tracy Community <br /> Hospital has re-initiated conversation with the Board of Equalization to apply for a BoE Account <br /> Number.
