This violation was corrected on site on <br /> 11/06/2015. Item completed <br /> 204 CCR 2712(i) Current monitoring plan approved by the EHD not found on <br /> site <br /> A copy of the monitoring plan was located and inserted into the binder. This <br /> violation was corrected on site on 11/06/2015. Item completed <br /> 208 CCR 2715 Designated operator monthly inspection reports not maintained <br /> on site for at least 12 months <br /> The February 2015 designated operator monthly inspection report could not be <br /> located. The service company provided the facility with a copy. This violation <br /> was corrected on site on 11/06/2015. Item completed <br /> 209 CCR 2715(l)Designated operator employee training not performed or log <br /> not kept <br /> Designated operator employee training performed on 11/06/2015 which was <br /> one month out of the once every 12 month mandatory requirement. Our facility <br /> has programmed a work order,for this training, scheduled to be generated <br /> annually in September. This will enable us to produce completed employee <br /> training documents prior to the scheduled annual inspection from the EHD. This <br /> violation was corrected on site on 11/20/2015 with a work order being entered <br /> into our computer for staff training scheduled next September.
