The following items need to be addressed prior to permit <br />approval: <br />1. Obtain An EPA # for the tank removal. <br />2. Complete Page #3 of the closure application pertaining to <br />the contractor. <br />3. You need to identify the laboratory name and phone #. <br />4. You need to identify the sampling firm and phone #. <br />5. What date were the tanks installed? <br />6. The applicant needs to sign, title, and date the bottom <br />of Page 13. <br />7. Please complete Page #4 items #1 (a), (b), (c). <br />8. Please submit a "Site Health & Safety Plan" <br />9. Please note the following dry ice requirements: <br />a) 15 lbs. per 1,000 gallon capacity for diesel tanks. <br />b) 30 lbs. per 1,000 gallon capacity for gasoline <br />tanks. <br />10. Who will be the contaminated soil hazardous waste hauler? <br />11. The owner/operator needs to sign and date page #9. <br />12.
