Each person installing or testing any component must hold a certificate of <br />training from the manufacturer of that component. <br />6. Contact Aris Cacapit (468-3453) to arrange the following inspection(s) (a 48-hour notice <br />is required): <br />a. If a "cold start" of the UST monitoring system is performed, the EHD must <br />witness the functionality of the monitoring system to ensure that monitoring has <br />been restored to meet the requirements of California Code of Regulations Title <br />23 and the Health & Safety Code. <br />7. Since concrete is being broken near the UST product lines, pipeline secondary <br />containment testing must be performed by a SWRCB licensed tank tester in accordance <br />with the manufacturer's guidelines. Results must be received by the EHD within seven <br />days of returning the facility to operating condition. <br />8. An "Underground Storage Tank Certification of Installation/Modification" form (form C) <br />shall be submitted by the owner or the owners' agent [pursuant to (CCR) 2635(d)] before <br />this upgrade will be considered complete by EHD. <br />9. Permit expires 180 days from date of approval.
