Victor Rd. <br />Lodi, CA 95240 <br />Mr. Cacapit, <br />On May 11, 2009 BZ Service Station Maintenance was at the above listed site to install <br />new Veeder Root DIM Communication Boards in existing tank monitor system for the <br />new Arco style POS console. This was needed to record and transmit fuel amounts to <br />Veeder Root for ISD tracing of vapors. Our technician had to archive old alarm history of <br />Veeder Root and re-enter it after a cold start. While doing so, the oldest alarm history <br />was dated March 26, 2009. It did not show a monitoring certification that was done by <br />another company for the EVR upgrade in February 2009. <br />BZ Service Station Maintenance is requesting that we add the cold start that was needed <br />on May 11, 2009 to the existing permit. <br />I have included copies of the tapes that were printed prior to the archive for your review. <br />If you have any questions, please contact Paul Ensele or myself at (916) 371-2380. <br />Sincerely, <br />Kim Hem <br />
