The maintenance record of 12-22-08 you <br />submitted does not address the L4 87 annular alarm that occurred on 12-31-08. 1 still need you to submit <br />documentation of Designated Operator provided training for Sukh Singh and Gurder Singh. Complete and <br />submit the attached DO UST Training form.
