Item #9 on the application <br />Questionnaire has been revised accordingly. A copy of the corrected questionnaire is <br />enclosed. <br />3) A site plan, on which the existing monitoring wells are located, has been <br />enclosed for your reference. <br />4) The Health & Safety Plan has been revised to incorporate your current <br />address & phone number. A copy of the revised page is enclosed for your <br />use. <br />INSTALLATION PLAN <br />1) A (TLM) Tank Level Monitor will be installed in the existing waste oil tank. <br />2) The Fernco Boot will not be used. Refer to details 5 & 6 on sheet F-1 which have been <br />revised. The secondary containment pipe ends at the connection to the dispenser sump <br />box where a liquid sensing probe is to be installed. These probes will be connected to the <br />CEI 3000 alarm panel. <br />3) The primary piping is A.O.
