Site EPA number <br />2. Decontamination information. Will pipes be <br />decontaminated prior to removal? <br />For installation: <br />1. Site EPA number <br />2. Cut sheet on dispenser pans. Are they compatible with <br />the product? <br />3. Information on mechanical leak detectors. The <br />application shows the installation of mechanical leak <br />detectors, the plan shows no leak detector. <br />4. Information on flex hose. The plan shows Teleflex, the <br />cut sheet shows Hose Master. <br />5. Complete question 18 and 19 (E) of the application. See <br />attached. Those questions refer to piping, not tank. <br />Please submit the above information to EHD. The application for <br />permit to close and to install will not be approved without those <br />information.
