The permit has been approved with the <br /> following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Garrett Backus at (209) 468-2986 to arrange the following inspection <br /> appointments (a 48-hour notice is required for inspection appointments). <br /> a. Overfill prevention verification: Measurement of drop tubes to verify 95% tank volume <br /> shut off. - <br /> b. Spill bucket test: Verify minimum 5-gallon capacity and pass a one hour hydrostatic <br /> test (or equivalent). Proper drainage must also be verified. <br /> c. 24-hour hydrostatic test (or equivalent) on all repaired sumps, after returning facility <br /> to operating condition. <br /> d. Secondary containment testing on all product tanks and product piping, after <br /> returning facility to operating condition. This testing is required since concrete will be <br /> broken over the product piping and tanks. Penetration fittings may also be replaced. <br /> Please be aware that the facility cannot operate the UST system or receive fuel in <br /> the 87 or 91 octane tanks prior to the EHD inspection and approval. <br /> 4. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), or a <br /> Tank Tester's license issued to the individual by the State Water Board, and <br />
