SA NA 0 A QU I N Environmental Health Department <br /> COUNTY <br /> c. Possesses a valid, current certificate of training from the manufacturer(s) of the <br /> component(s) being tested, repaired, or serviced. <br /> 5. Individuals installing underground storage tank system components must possess or work <br /> under the direct and personal supervision of an individual physically present at the work site <br /> who possesses all of the following: <br /> a. A valid, current contractor's license issued by the Contractors State License Board, and <br /> b. A valid, current UST Installation/Retrofitting certificate issued by the International Code <br /> Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s) of the component(s) being <br /> installed. <br /> 6. Log into the California Environmental Reporting System (CERS), update and submit the "UST <br /> Monitoring Plan" for tank three for approval. <br /> 7. Log into the California Environmental Reporting System (CERS), complete and submit a "UST <br /> Certification of Installation/Modification" form for the UST within 30 days. <br /> 8. All components must be certified by an independent testing organization and labeled as <br /> required by the organization (UL, ULC, ANSI, API, ASTM, etc.). See California Code of <br /> Regulations (CCR) Title 23, Chapter 16, Table B for a complete list. <br /> 9. Maintain continuous system monitoring (as applicable) during all upgrade activities. <br /> 10. All installation and testing shall be in compliance with the manufacturer's instructions, section <br /> 2635 and section 2636 of California Code of Regulations (CCR). <br /> 11. An accurate and drawn to scale as-built plan must be submitted to the EHD prior to scheduling <br /> a final inspection. <br /> 12. No fuel or other hazardous material or waste can be released (intentionally or unintentionally) <br /> to the sewer/septic, storm water system, or any other waters of the state. If a release occurs it <br /> will be considered illegal hazardous waste disposal and will be subject to enforcement. <br /> 13. Submit to CERS an accurate Monitoring Plan prior to scheduling your final inspection. <br /> 14. Permit expires 180 days from the date of approval.
