Please complete the <br />following items within 30 days of the close of escrow [CA Health and Safety Code section 25284 (b) & (c)]: <br />1. Report#5021: Update all information for all programs at this site including the UST, Hazardous Waste, <br />Hazardous Materials and Food programs, sign and date. Bring all forms to our accounting office to pay for <br />transfer fees and the $ l30/hr UST Consulting fee, if necessary. <br />2. Create an account with the CA Environmental Reporting System (CERS) by going to <br />http://cers.calepa.ca.gov. Complete the enclosed CERS Lead User Authorization Form and return to the <br />EHD. Request access to existing facility information in CERS. <br />3. Underground Storage Tanks Facility Form, Tank Forms, and Monitoring Plan must be filled out and <br />submitted in CERS. <br />4. Certification of Financial Responsibility/Letter from Chief Financial Officer: Complete the forms in CERS <br />and use wet signatures. This document is required to be submitted into CERS annually. <br />5. Underground Storage Tank Response Plan: Complete, sign and submit a plan into CERS. <br />6. Fill out and submit the Designated UST Operator Statement and submit into CERS. <br />7 Go to: http://www.waterboards.ca. o�phome/ust/re ug lator_y/index.html - rgs to obtain the current statutes <br />(Underground Storage of Hazardous Substances) and UST Regulations that you are required to have for your <br />monitoring and response plan. <br />8 Also enclosed for your information is a brochure of Best Management Practices (BMP's) for Retail Gasoline <br />Outlets that you can use as part of your BMP's for your facility. <br />9 Hazardous Materials Management Plan: Update this information and submit in CERS. Contact EHD Hazmat <br />Staff Haza Saeed at (209) 953-6212 for information on updating this plan. <br />10 Information and instructions for obtaining a permanent California EPA ID number: Complete the application <br />for a California Hazardous Waste Permanent ID Number and fax it to (916) 255-4703. <br />
