The permit has been approved with the following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Thuy Tran at (209) 468-8257 to arrange the following inspection appointments <br />(a 48-hour notice is required for inspection appointments). Please be aware that <br />inspections for emergency work must be scheduled within five days of permit approval. <br />a. Secondary containment testing on all UDC sumps and product piping. <br />Please be aware that the facility cannot operate the UST system prior to the EHY <br />inspection and approval. <br />4. Secondary containment testing for the UDC sumps and product piping are required six <br />months after installation. Provide 48 hours notification to the EHD when this test is <br />scheduled. <br />5. Log into the California Environmental Reporting System (CERS), complete and submit a <br />"UST Certification of Installation/Modification" form for the penetrations installed in the <br />UDCs. <br />6. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />
