The permit has been approved with the following conditions: 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any applicable permits and requirements. - a. Contact Michelle Henry to arrange the following inspections(a 48 hour notice is required): Functionality test to ensure that monitoring has been restored to meet the requirements of California Code of Regulations Title 23 and the Health&Safety Code. 2._ Qualified service technicians must possess or work under the direct and personal supervision of an individual physically present at the work site who possesses all of the following: a. One of several permissible valid,current contractor's licenses issued by the Contractors State License Board ma be issued to the installation company) (may p._..Y�� -- b. A valid,current California UST Service Technician certificate issued by the International Code Council(ICC),and - __ c. A valid,current certificate of training from the manufacturer(s)of the component(s)being tested,repaired,or serviced,and d. Each person installing or testing any component must hold a certificate of training from the manufacturer of that component. 3. Permit expires 180 days from date of approval. Note: If an extension to the expiration date is required,a written request must be submitted to this office prior to the expiration date. 4 _All components must be certified by an independent testing organization and labeled as required by the organization. (UL,ULC,ANSI, API,ASTM,etc. See California Code of-- -Regulations f—-Regulations(CCR),Title 23 Chapter 16 Table B for complete list).
