Also enclosed are copies of the financial <br />assurance documents, the updated Owner Statement of Designated UST Operator <br />form, documentation of attendant training, and the most recent monthly inspection. <br />Please note that the Dispenser Containment Box failed the test. The box will be <br />repaired, retested and the County will be notified of the results.
