2. PLAN REVIEWOPERATING PERMIT <br />a. New facility plan review fee of $696.00 (8 hour minimum per facility), $500.00 facility fee <br />(includes one tank), -$125.00 per tank after the first tank, plus $8.00 state surcharge fee per <br />tank is due at time of plan submittal plus $18.50 per site state surcharge. <br />b. New installation of an additional tank at an existing facility is $696.00 plan review fee (8 hour <br />minimum per facility) plus $500.00 facility (includes one tank), $125 per new tank after first <br />tank, and $8.00 per new tank state surcharge fee. <br />C. It should be noted that a $696.00 plan review check fee required for each submittal allows for <br />up to eight (8) hours of staff review and/or inspection time. Additional time spent on plan <br />review, meetings, inspections, phone or office consultations will be billed to the applicant at a <br />rate of $87.00 per hour. <br />d. The maximum review time for UST installation plans is 40 working days. A preliminary <br />review will be conducted by the PHS-EHD within 20 working days of submittal date to <br />identify any gross plan deficiencies. <br />e. If gross deficiencies are identified, the plans will be returned with a checklist identifying plan <br />deficiencies. <br />To expedite the review of installation plans, time and a half per hour overtime fee may be <br />submitted to have review completed within 10 working days of submittal date if staff is <br />available to perform the review. <br />3. PLAN REVIEW GUIDELINES <br />a. The enclosed plan review sheet is to be completed and submitted with the three sets of <br />drawings to PHS-EHD. <br />b. A copy of the California UST Regulations may be obtained from the California State Water <br />Resources Control Board at (916) 227-4303. <br />C. Permits and/or plan check approval may also be required by the San Joaquin Valley Unified <br />Air Pollution Control District, the County/City Building Department and the Fire District in <br />your respective area. <br />d. A minimum of 3 inspections will be conducted on the UST system installation. A $87 per <br />hour inspection fee shall be charged for additional inspections or reinspection as required. <br />(See required inspections below) <br />e. Applicant/contractor to adhere to the approval plan and any change or deviation shall be <br />reported to and approved by PHS-EHD prior to installation. <br />f. The application for installation of underground tanks is only valid for the calendar year in <br />which it has been issued. A permit may be extended into the next calendar year if a letter is <br />sent to PHS-EHD requesting this extension thirty days prior to the end of the calendar year. A <br />one time, one year extension may be granted by PHS-EHD upon receipt of this letter. <br />2 <br />
