Ad Art, Stockton Inspection Jan 30, 2012 <br />Name: g y Address: Date: <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS II, or MINOR -Notice to Comply) <br />This is an annual routine and monitoring system certification inspection. The following is a Minor -Notice <br />to Comply: #313 <br />313. The spill container failed when tested. All spill containers shall have a minimum capacity of five <br />gallons and be capable of containing a spill or overfill until it is detected or cleaned up. Immediately <br />contact a properly licensed, trained, and certified contractor to repair or replace the spill container under <br />permit and inspection of the EHD. Submit a permit application to EHD by March 2, 2012. <br />NOTES <br />All four sensors were tested by designated operator GEMS on May 12, 2011 and "approval for use of an <br />extension cord" to remove liquid from the fill sump according to May 2011 designated operator report <br />and Jim Murray during interview. EHD was not notified of the work. Based on this inspection, no cold <br />start was implemented as a result of the work conducted on May 12, 2011. Please be aware that any <br />repair work or work that could affect the functionality of the monitoring system must be done under <br />permit from EHD. <br />The expiration date of the designated operator Raymond Friend on the January 17, 2012 designated <br />operator monthly report was January 15, 2012. In a telephone conversation Raymond stated the <br />expiration date on the report was in error as he had renewed his certification. Raymond stated he will be <br />submitting evidence of his current certification expiration date to this office.
