Hwy 88 <br /> Lodi, CA 95240 <br /> The San Joaquin County Environmental Health Department(EHD) has completed the review of <br /> the application for a permit to install a direct bury 1 C-2100C-DEVR OPW 5 gallon spill container <br /> on the diesel underground storage tank fill riser at the above noted address. The permit has <br /> been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Aris Cacapit at(209) 468-3453 to arrange the following inspection appointments <br /> (a 48-hour notice is required for inspection appointments). Please be aware that <br /> inspections for emergency work must be scheduled within five days of permit approval. <br /> a. Corrosion protection: Verify corrosion protection for direct bury spill container prior to <br /> backfilling. <br /> b. Spill container test: Verify minimum 5-gallon capacity and pass a one hour <br /> hydrostatic test(or equivalent). Proper drainage must also be verified. <br /> c. Secondary containment testing on the diesel tank. <br /> Please be aware that the facility cannot receive fuel in the diesel tank prior to the <br /> EHD inspection and approval. <br /> 4. Since concrete will be broken near the diesel tank, a secondary containment test must <br /> be performed on the diesel tank once concrete is poured in accordance with the <br /> manufacturer's guidelines. <br />
