Contact the local fire department and San Joaquin Valley Air Pollution Control District for any <br />applicable permits. <br />2. Any changes shall be submitted to this office for approval prior to commencing work. All new <br />addenda supercede the previous ones. <br />3. At least one person who holds a current International Code Council (ICC) UST Service Technician <br />certification and manufacturers' certificate of training for all components installed or tested must be <br />onsite when work is performed. <br />4. Each person installing or testing any component must hold a certificate of training from the <br />manufacturer of that component. <br />5. Contact this office for the following inspections: <br />a. One hour soap test of supreme unleaded product line. <br />b. 24 hour or equivalent lake test of dispenser pan 1/2. <br />6. Allow 48-hour notice for all appointments. <br />7. Once concrete is poured, all lines shall have an integrity test performed in accordance to manufacturer. <br />Results must be received by EHD within 7 days of returning the site to operating condition. <br />8. Complete and submit a copy of a UST Installation Certificate of Compliance (Former Form C) for <br />each tank no later than 30 days after work is completed. <br />9.
