An alarm log explaining the following alarms was not found on site: L8 - tank annular, setup data <br /> warning (2-21-06), sensor out alarm (2-21-06), and fuel alarm (2-21-06). These records shall be kept on <br /> site for at least 3 years. Immediately locate and maintain the alarm log for the above alarm on site and <br /> submit a copy to the Environmental Health Department (EHD) by May 25, 2011. Explain why the most <br /> recent L-8 sensor alarm history has the following alarms: setup data warning (2-21-06), sensor out <br /> alarm (2-21-06), and fuel alarm (2-21-06). <br /> 7. The 2-23-10 secondary containment test results and the 5-17-10 monitoring system certification test <br /> results were not found on site. These records shall be kept on site for at least 3 years. The 2-23-10 <br /> secondary containment test results were provided to the operator by the inspector. Immediately locate <br /> and maintain the 5-17-10 monitoring system certification test results on site and submit a copy to the <br /> Environmental Health Department (EHD) by May 25, 2011. <br /> 12 & 16. The 91 octane secondary piping test boot was attached to the product piping entering the 91 <br /> turbine sump. The test boot prevented piping leaks from draining back to the sump. Any potential <br /> release from the primary containment into the secondary containment in the 91 piping did not flow into a <br /> collection sump. All secondary containment systems shall be sloped so that all releases will flow to a <br /> collection sump located at the low point of the underground piping. The testing technician removed the <br /> test boot, during the inspection, to ensure that any releases in the 91 piping flows into the 91 turbine <br /> sump. Corrected during the inspection. <br /> 13. The monitoring panel failed to activate a visual alarm when tested, due to a burned out light on the <br /> panel. All monitoring equipment shall be maintained to activate an audible and visual alarm. Correct <br /> immediately by having a properly licensed, trained, and certified contractor replace the failed component <br /> with a functional component (LG 113-x listed, if applicable) and obtain a permit within 1 business day <br /> from the EHD. If the failed component can't be replaced immediately, there is a possibility that the UST <br /> system may be red tagged to prevent fuel inputs. <br /> 24. Annual monitoring system certification, leak detector testing, and spill container testing was <br /> performed on May 17, 2010 and a test report has not been submitted to the EHD. A copy of the test <br /> report must be submitted within 30 days of the tests. Immediately submit a copy of the test report to the <br /> EHD. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FAAILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD or: Received B Title: <br /> O 3-02-003 Rev 08/10/10 CONTINUATION FORM <br />
