The permit has been approved with <br /> the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any <br /> applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Stacy Rivera at(209)468-3440 to arrange the following appointments(a 48-hour notice is <br /> required for inspection appointments). <br /> • Sensor functionality: Functionality of the monitoring system must be verified to ensure that <br /> monitoring has been restored to meet the requirements of Title 23 California Code of Regulations <br /> and the Health &Safety Code. If it is determined that a cold start is necessary, a copy of the <br /> alarm history shall be printed prior to performing the"cold start"of the monitoring system and <br /> made available at the time of EHD inspection. <br /> Please be aware that the facility cannot operate the UST system prior to the EHD inspection <br /> and approval. <br /> 4. If concrete will be broken near the tank, a secondary containment test must be performed on the <br /> tank once concrete is poured in accordance with the manufacturer's guidelines. <br />
