Board of Equalization UST Storage Fee Account Number—Call (916) 322-9669 to <br /> obtain a current number for your facility. <br /> 12. San Joaquin Valley Air Pollution Control District—Call (209) 557-6400 to obtain an <br /> operating permit for the UST vapor recovery system. <br /> 13. Food facility operations—Call Vidal Pedraza at (209) 468-0334 for operating permit <br /> information for opening the retail food facility. A consultation appointment may be <br /> needed. <br /> 14. Annual monitoring system certification, leak detector testing, and spill container testing <br /> were last performed on August 5, 2014. These tests are required once every 12 <br /> months. The next inspection has been scheduled for July 28, 2015. <br /> 2. Secondary containment testing was conducted on May 20, 2013. Secondary <br /> containment testing is required once every 36 months.
