A copy of the test report must be submitted within 30 days of the <br /> tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely <br /> manner. <br /> This is a Class II violation. <br /> 308 CCR 2636(f) Failed to maintain a continuous monitoring system that activates an audible and visual alarm. <br /> The diesel south piping sensor failed to activate an audible and visual alarm when tested to ensure continuous <br /> monitoring of the tank system. All monitoring equipment shall be maintained to activate an audible and visual alarm or <br /> stop the flow of product at the dispenser when it detects a leak. The sensor was replaced and retested during the <br /> inspection. A"UST Retrofit Verification with Inspector Already Onsite" form has been completed and provided to the <br /> operator and contractor. <br /> This is a minor violation. <br /> 309 CCR 2636(f)(1) Monitoring in the UDCs failed to activate an audible and visual alarm or stop the flow at the UDC. <br /> The Bravo box float and chain in the UDC 5/6 satellite sump failed to stop the flow of product at the dispenser when <br /> tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of <br /> product at the dispenser when it detects a leak. The service technician adjusted the float and verified functionality. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> An inspection report was provided to the facility operator on the day of inspection. The report was amended by <br /> the EHD. The amended report replaces the initial report. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, within 30 days of receiving the amended inspection report. <br /> Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br /> documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br /> Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer (if <br /> applicable), and the Owner Statement of Designated Operator Compliance. The UST Owner/Operator: Written <br /> Agreement, if applicable, can be stored at the facility or uploaded into CERS. <br /> Documents reviewed: CERS submittals, alarm history, monthly designated operator inspection reports, <br /> designated operator training, maintenance records, test results <br /> Documents provided: Return to Compliance Certification form <br /> Page 4 of 5 <br />
