Notification shall be provided to the EHD at least 48 hours in <br /> advance of testing. The EHD will not accept these results and testing must be rescheduled with a 48-hour notification. <br /> Testing was last performed on January 12, 2012 for UDC 1/2 and on March 17, 2015, but failed, for the 87 piping, and <br /> was due by March 2015. Immediately schedule this test and provide 48 hours notification to the EHD. <br /> This is a Class II violation. <br /> 110 CCR 2637(e) Failed to submit secondary containment test report within 30 days. <br /> Secondary containment testing was performed on February 4, 2016 and a test report was submitted during today's <br /> inspection, on March 11, 2016, one month late. A copy of the test report must be submitted within 30 days of the <br /> tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely <br /> manner. <br /> This is a Class II violation. <br /> 308 CCR 2636(f) Failed to maintain a continuous monitoring system that activates an audible and visual alarm. <br /> The diesel south piping sensor failed to activate an audible and visual alarm when tested to ensure continuous <br /> monitoring of the tank system. All monitoring equipment shall be maintained to activate an audible and visual alarm or <br /> stop the flow of product at the dispenser when it detects a leak. The sensor was replaced and retested during the <br /> inspection. A"UST Retrofit Verification with Inspector Already Onsite"form has been completed and provided to the <br /> operator and contractor. <br /> This is a minor violation. <br /> 309 CCR 2636(f)(1) Monitoring in the UDCs failed to activate an audible and visual alarm or stop the flow at the UDC. <br /> The Bravo box float and chain in the UDC 5/6 satellite sump failed to stop the flow of product at the dispenser when <br /> tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of <br /> product at the dispenser when it detects a leak. The service technician adjusted the float and verified functionality. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by April 10, 2016. <br /> Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br /> documents into the California Environmental Reporting System (CERS):UST Monitoring Site Plan, UST <br /> Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer(if <br /> applicable), and the Owner Statement of Designated Operator Compliance.
