Secondary containment testing of UDC 1/2 and the 87 piping was performed on February 4, 2016 without providing notification to the EHD at least 48 hours in advance. Notification shall be provided to the EHD at least 48 hours in advance of testing. The EHD will not accept these results and testing must be rescheduled with a 48-hour notification. Testing was last performed on January 12, 2012 for UDC 1/2 and on March 17, 2015, but failed, for the 87 piping, and was due by March 2015. Immediately schedule this test and provide 48 hours notification to the EHD. This is a Class II violation. 110 CCR 2637(e) Failed to submit secondary containment test report within 30 days. Secondary containment testing was performed on February 4, 2016 and a test report was submitted during today's inspection, on March 11, 2016, one month late. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. This is a Class II violation. 308 CCR 2636(f) Failed to maintain a continuous monitoring system that activates an audible and visual alarm. The diesel south piping sensor failed to activate an audible and visual alarm when tested to ensure continuous monitoring of the tank system. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. The sensor was replaced and retested during the inspection. A"UST Retrofit Verification with Inspector Already Onsite"form has been completed and provided to the operator and contractor. This is a minor violation. 309 CCR 2636(f)(1) Monitoring in the UDCs failed to activate an audible and visual alarm or stop the flow at the UDC. The Bravo box float and chain in the UDC 5/6 satellite sump failed to stop the flow of product at the dispenser when tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. The service technician adjusted the float and verified functionality. This is a minor violation. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by April 10, 2016.
