This <br />information was required to be submitted by September 6, 2009. The requested information still has not been <br />received resulting in a non-compliant status for this facility. <br />Failure to correct the violations will result in continuing/additional violations. <br />Note: All EHD staff time associated with failing to comply, including the issuance of this letter, is billed at the <br />current hourly rate ($115).
