Lockeford St. Lodi, CA 95240 <br />Program: UST <br />SUMMARY OF VIOLATIONS <br />CLAN 1 CLASS II or MINOR -Notice to Com <br />On site to conduct a routine inspection and witness the annual monitoring system certification testing. <br />#2 Repairs to the failed 89 line secondary containment tested to pass on 9-21-09 and the six month <br />retesting was due by 3-21-10. Testing has not been completed and is 17 months past due. Secondary <br />containment systems installed on or after January 1, 2001 shall be tested upon installation, 6 months <br />after installation, and every 36 months thereafter. Correct immediately by scheduling a licensed, <br />certified, and trained contractor to complete the secondary containment six month retest of the repaired <br />89 line. This office must be provided with a testing notification at least 48 hours in advance. <br />13. The 87 product Bravo box float and chain in the Dispenser #1/2 sump failed to stop the flow of <br />product at the dispenser when tested. All monitoring equipment shall be maintained to activate an <br />audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. The service <br />technician adjusted the chain and verified functionality. Corrected on site. <br />18. The 87 spill container in the fill sump without a sump sensor failed when tested. All spill containers <br />shall have a minimum capacity of five gallons and be capable of containing a spill or overfill until it is <br />detected or cleaned up. The spill container was replaced and retested during the inspection. Obtain a <br />permit from the EHD for this work within one business day. If a permit is not obtained within one <br />business day, a "Failure to Obtain a Permit" penalty will apply. <br />21. Liquid was observed in the 89 & 91 turbine sumps. If water could enter into the secondary <br />containment by precipitation or infiltration, it must be removed and disposed of properly. Technician <br />removed the liquid and placed it into the hazardous waste drum on site. Ensure that the sumps are <br />maintained free of liquid.
