An incorrect document was uploaded in its place. A site <br /> plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the <br /> California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/and upload a copy of the UST <br /> Monitoring Site Plan. <br /> This is a minor violation. <br /> 209 CCR 2715(f) Designated operator employee training not performed or log not kept. <br /> The designated operator employee training for Doug Smith was not performed. The designated operator shall train <br /> facility employees for which he or she is responsible in the proper operation and maintenance of the UST system once <br /> every 12 months. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s)for emergencies and monitoring equipment alarms <br /> Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of <br /> the training records to the EHD. <br /> This is a repeat violation, Class II. <br /> 325 CCR 2635(b) Spill container failed to contain a minimum capacity of five gallons. <br /> The 87 direct-bury spill container failed when tested. All spill containers shall have a minimum capacity of five gallons <br /> and be capable of containing a spill or overfill until it is detected or cleaned up. Immediately discontinue deposition of <br /> petroleum into this tank until the component is repaired or replaced by a properly licensed, trained, and certified <br /> contractor under permit and inspection of the EHD. If the spill container can't be replaced immediately, there is a <br /> possibility that the 87 UST system may be red tagged to prevent fuel inputs. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by May 14, 2015. <br /> "This inspection report was revised on April 27, 2015 to add violation #325. This violation was added based on an <br /> application for a permit to replace the 87 spill container, received on 4/23/15, which stated that the spill container <br /> had to be replaced "due to fill bucket lake test failure."
