Current financial responsibility documents have not been submitted to the EHD. The most recent <br /> copy submitted to EHD was valid until 4-15-14. Current financial responsibility documents are required <br /> to be submitted annually. Immediately submit a copy of the financial responsibility documents to the <br /> EHD. Alternatively, log into CERS, upload the required information, and submit for review by the EHD. <br /> Complete by 5-16-14. <br /> 201. Maintenance records for the L4 87 annular alarms on 8-16-13 and 6-30-13 were not found on site <br /> nor did the designated operator document what was done in response to these alarms. These records <br /> shall be kept on site for at least 3 years. Ensure that maintenance records for alarms are kept on site. <br /> 204. The designated operator failed to document responses to the alarms from the attached alarm <br /> history on the 8-28-13 and 7-29-13 designated operator monthly inspection reports. The missing alarms <br /> include: 8-16-13 and 6-30-13 L4 87 annular sensor alarms. The 5-29-13 designated operator report did <br /> not have attached a copy of the alarm history. During the monthly inspection, the designated operator <br /> shall review the alarm history for the previous month, check that each alarm was documented and <br /> responded to appropriately, and attach a copy of the alarm history with documentation taken in response <br /> to any alarms to the monthly report. Ensure that designated operators performing monthly inspections <br /> at this facility are including all of the required information on the reports. <br /> 303. Approximately one cup of liquid is at the eastern bottom of the 87 piping sump. The 87 piping <br /> sump sensor was located above the liquid in the sump approximately one inch above the lowest point of <br /> the sump and not located to detect a leak at the earliest opportunity. Monitoring equipment shall be <br /> maintained to be able to detect a leak at the earliest possible opportunity. During the inspection, the <br /> technician adjusted the hanger of the sensor and repositioned the sensor so that it reaches the lowest <br /> point of the sump to detect a leak at the earliest opportunity.
