Hazelton Avenue ENVfRONMENTAL HEALTH <br /> Stockton, CA 95202-6232 PERMIT/SERVICES <br /> STATEMENT OF CORRECTIVE ACTION TAKEN AND PROOF OF RETURN TO COMPLIANCE FOR <br /> CITY OF STOCKTON MUNICIPAL SERVICE CENTER—4/18/13 UST INSPECTION <br /> We hereby submit our statement of corrective action taken by the City of Stockton Municipal Service <br /> Center in relation to UST violations cited on 4/18/13 by SJC EHD. <br /> Item #104: The Certificate of Financial Responsibility from our Chief Financial Officer is enclosed <br /> along with the Certification of Financial Responsibility form. <br /> Items#108 and #303: Items were corrected on-site at the time of the inspection as noted in the <br /> enclosed copy of the Summary of Violations. <br /> Item #306: Fleet's contracted fuel vendor inspected the UDC area for leaks and none were found. <br /> The fuel vendor cleaned the area and rechecked for leaks. See enclosed email. <br /> Miscellaneous Note: Please see UPCF-UST Form-C dated 5/15/13 which reflects the updated <br /> scope of work for the December 2012 UDC repairs performed by our fuel vendor.
