Financial responsibility documents have not been submitted to the EHD. The most recent financial <br />responsibility forms on file are dated 4/7/11. Current financial responsibility documents are required to <br />be submitted annually. Complete and submit a copy of the financial responsibility by May 18, 2013. <br />108. Secondary containment testing was performed on January 3, 2013 and a test report has not been <br />submitted to the EHD. A copy of the test report must be submitted within 30 days of the test. A copy of <br />the test report was provided by the facility at the time of inspection. <br />303. The sensors in UDCs 1/2 and 3/4 were not placed in the indented area of the dispenser pan <br />designed as the lowest point of the sump and not located to detect a leak at the earliest opportunity. <br />Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. <br />The sensors were relocated at the time of inspection. <br />306. A small amount of liquid was observed in UDC 7/8. All primary containment must be product tight. <br />Immediately determine the source of the leak and discontinue use of any failed piping. If necessary, <br />have a properly licensed, trained, and certified contractor replace or repair the failed piping under permit <br />and inspection of the EHD. If the failed component can't be replaced immediately, there is a possibility <br />that the diesel UST system may be red tagged to prevent fuel inputs. <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD by May 18, 2013 <br />along with a statement documenting the corrective actions that have been or will be taken for each <br />violation, and any supporting paperwork. <br />Note: Form C and updated scope of work needed for December 2012 UDC repairs. <br />Provided: Return to Compliance Certification form <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />
