Hazelton Avenue <br />Stockton, CA 95202-6232 <br />STATEMENT OF CORRECTIVE ACTION TAKEN AND PROOF OF RETURN TO COMPLIANCE <br />FOR CITY OF STOCKTON MUNICIPAL SERVICE CENTER — 4/11/12 UST INSPECTION <br />We hereby submit our statement of corrective action taken by the City of Stockton Municipal <br />Service Center in relation to UST violations cited on 4/11/12 by SJC EHD. <br />Item #102: Invoice #0223338 for the $42.00 late fee penalty was paid on 5/10/12. See <br />Attachments Al -A2. UST Operating Permit to be picked up by Fleet personnel on 5/11/12. <br />Item #109: Test results were withheld by Afforda-Test due to non-payment by Stockton <br />Service Station. Upon receipt of payment by City of Stockton, Afforda Test forwarded test <br />results to SJC EHD. See Attachments 131-136. <br />Item #109 & #111: Annual Monitoring System Certification test was performed on 4/18/12. <br />Annual Spill Container test was performed on 4/17/12.
