Annual operating fees for the 2012 operating year have not yet been paid. Operating fees must be <br />paid the San Joaquin County Environmental Health Department (EHD) on an annual basis. Immediately <br />contact the EHD to arrange for payment of these fees. <br />109. Annual monitoring system certification and spill container testing was performed on March 10, 2011 <br />and a test report has not been submitted to the EHD. A copy of the test report must be submitted within <br />30 days of the tests. Immediately submit a copy of the 2011 test report to the EHD. <br />109 & 111. Annual monitoring system certification and spill container testing were last performed on <br />March 10, 2011 and was due by March 2012. Testing has not been completed and is one month past <br />due. These tests are required once every 12 months. Immediately schedule these tests and provide 48 <br />hours notification to the EHD. Please be aware that a follow up inspection will be charged at our current <br />hourly rate. <br />Complete and submit a copy of the Return to Compliance Certification to the EHD by May 11, 2012 <br />along with a statement documenting the corrective actions that have been or will be taken for each <br />violation and any supporting paperwork. <br />Provided: Return to Compliance Certification form <br />ALL END STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />
