Main Street <br />Stockton, CA 95202-3029 <br />RECEIVED <br />APR 07 2011 <br />ENVIRONMENTAL HEALTH <br />PERMIT/SERVICES <br />CORRECTIVE ACTION TAKEN AND PROOF OF RETURN TO COMPLIANCE <br />We hereby submit our statement of corrective action taken by the City of Stockton Municipal <br />Service Center in relation to UST violations cited on 3/10/11 by SJC EHD. <br />Item #21 — Code Section HSC 25291 (e): Water in 2 containment (diesel piping sump) not <br />removed, analyzed and properly disposed of. Action Taken: Removed by technician on <br />site. We also cleaned the lip area of lid to provide a better seal. Attached pictures of area. <br />Item #23b — Designated operator did not possess a current Operator Certification. Action <br />Taken: We hired Karen Arnaiz to be our designated operator. Updated Designated UST <br />Operator Form attached with copy of UST System Operator Certificate. Operator Karen <br />Arnaiz conducted our Monthly Visual Inspection. Checklist attached. <br />Item #23d — Designated operator employee training: Action Taken: Our new Operator <br />Karen Arnaiz trained 12 employees on March 30, 2011.
