If you need to review our file(s), a public records release will <br />need to be submitted and you can make an appointment to review and/or copy the applicable information to help satisfy <br />this requirement. I will do my best to review the documentation that you present in a timely manner. I have enclosed a <br />copy of the service request form for your conveinance.
