If you need to <br />review our file(s), a public records release will need to be submitted and you can make an <br />appointment to review and/or copy the applicable information to help satisfy this <br />requirement. I will do my best to review the documentation that you present in a timely <br />manner. I have enclosed a copy of the service request form for your conveinance. We <br />just became aware of this approval requirement, and would like to complete the approval <br />process as soon as possible.
