Copies of last submitted Facility and Tank Forms, which are correct. <br />2. Copy of invoice for permit to replace two shear valves. Elite IV replaced <br />on 5/18/07 and subsequently applied for permit. <br />3. No record of the Ll 91 STP fuel alarm being called in or Elite IV being <br />dispatched. Spurious alarm? <br />4. The carrier has been cautioned to not allow overfilling of tanks during fuel <br />drops. <br />5. Maintenance and monitoring records are maintained onsite, either attached <br />to the D.O.'s monthly inspection forms, in the blue UST manual, in the <br />site's safety manual, or in a binder provided by the D.O. I cannot account <br />for why they could not be located. <br />6. All hazardous waste is properly disposed under manifest. <br />7.
