The <br />permit has been approved with the following conditions: <br />1. Contact the local fire department, and the San Joaquin Valley Air Pollution Control <br />District for applicable permits and requirements. <br />2. Any changes in information or scope of work must be submitted to SJCEHD prior to <br />commencing work. All new addendums supercede the previous ones. <br />3. At least one person who holds the International Code Council "UST <br />Installer/Retrofitter" certification must be onsite AT ALL TIMES when work is <br />performed and manufacturers' certificate of training for all components installed or <br />tested must be onsite when work is performed. <br />4. Each person installing or testing any component must hold a certificate of training <br />from the manufacturer of that component. <br />5. The "Certificate of Compliance for Underground Storage Tank Installation Form C" <br />shall be submitted by the owner or the owners' agent [pursuant to California Code of <br />Regulation (CCR) 2635(d)] before this upgrade will be considered complete by <br />SJCEHD. <br />6. Contact Ray von Flue at (209) 468-9848 to arrange the following inspection <br />appointments; a 48-hour notice for inspection appointments is required. <br />a. Spill Bucket: verify minimum 5 -gallon capacity and conduct a 60 -minute lake <br />test in compliance with CCR 2635(b)(1). Proper drainage must also be verified. <br />7. Permit expires 90 days from the approval date.
