Maintenance record for the 8-5-10 L3 87 turbine sump sensor fuel alarm were not found on site. <br />These records shall be kept on site for at least 3 years. Immediately locate and maintain the <br />maintenance record for the 8-5-10 L3 87 turbine sump sensor fuel alarm on site and submit copies to the <br />EHD by 8-15-11. Repeat Violation. <br />13. The 91 Bravo box float and chain in the #1/2 dispenser sump failed to stop the flow of product at the <br />dispenser when tested. All monitoring equipment shall be maintained to activate an audible and visual <br />alarm or stop the flow of product at the dispenser when it detects a leak. The service technician adjusted <br />the chain and verified functionality. Repeat Violation. Corrected on site. <br />17. The 87, 89, and 91 line leak detectors failed to detect a leak when tested. All line leak detectors shall <br />be capable of detecting a 3 -gallon per hour leak at 10 psi. The service technician adjusted the leak <br />detectors, retested them, and they passed. Repeat Violation. Corrected on site. <br />18. The 87 spill container failed when tested. All spill containers shall have a minimum capacity of five <br />gallons and be capable of containing a spill or overfill until it is detected or cleaned up. Immediately <br />discontinue deposition of petroleum into this tank until the component is repaired or replaced by a <br />properly licensed, trained, and certified contractor under permit and inspection of the EHD. If the spill <br />container can't be replaced immediately, there is a possibility that the 87 UST system may be red tagged <br />to prevent fuel inputs. Repeat Violation. <br />21. Liquid was observed in the 87 and 91 turbine sumps. If water could enter into the secondary <br />containment by precipitation or infiltration, it must be removed and disposed of properly. During the <br />inspection, the technician removed the liquid and placed it into the hazardous waste drum on site. <br />Continue to ensure that liquid from the secondary containment is removed, a hazardous waste <br />determination is made per Title 22 hazardous waste regulations, and the liquid waste is managed <br />accordingly. Continue to ensure that the sumps are maintained free of liquid. Repeat Violation. <br />Corrected on site. <br />23c. The designated operator failed to visually inspect the 87 turbine sump for leaks, water, or debris <br />during the 9-2-10 monthly inspection. During the monthly inspection, the designated operator shall <br />visually inspect all spill containers and UDC sumps, and any tank top sump that had an alarm for which <br />there is no record of a service visit. Ensure that designated operators performing monthly inspections at <br />ALL EHD STAFF 11ME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THISfF CILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Received By: <br />Title: <br />EHD 23-02-003 Rev 08/10/10 CONTINUATION FORM <br />
