Lodi, CA <br />Program: UST <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS 0, or MINOR -Notice to Com <br />On site to witness the annual monitoring system certification testing and conduct the routine inspection. <br />Repeat Violations: # 18, 21, 23c. Violations: # 9, 13 <br />#9 Max product alarms occurred three times on June 7, 2009 at 11:36 AM, 12:01 PM, and 12:23 PM on <br />the 87 tank. The underground storage tank (UST) system must be operated to prevent unauthorized <br />releases, including spills and overfills. Correct immediately. <br /># 13 The float and chain for the 87 product in the under dispenser containment (UDC) did not shut off <br />the product valve at the dispenser when tested with water. Technician corrected during the inspection by <br />adjusting the chain and retested the float to pass the functionality test. <br />#18 The 89 and 91 OPW Pomeco spill containers in the fill sumps failed. Immediately discontinue <br />deposition of fuel into the 89 and 91 product tanks. Correct immediately by having a licensed, trained, <br />certified contractor repair/replace the failed component under permit from the San Joaquin County <br />Environmental Health Department. <br />#21 Approximately two quarts of water was in the bottom of the 87 submersible turbine pump (stp) <br />sump. Approximately one cup of water was in the bottom of the 91 stp sump. Approximately 12 gallons <br />of water was in the 89 fill sump. Approximately 30 gallons of water was in the 91 fill sump. Approximately <br />2 quarts of water was in the 87 fill sump. Water in the secondary containment must be properly disposed <br />of. Technician corrected on site by removing the water and disposing of it into the hazardous waste <br />drums on site. <br />#23c Designated Operator (D.O.) report 8-3-09 lists secondary containment testing as 11-26-09. It <br />should be 9-7-06. The D.O. report for December 2009 was not on site nor available for review.
